I authorise all medical treatment and/or hospital procedures as may be performed or prescribed by the attending physician and/or paramedics for my child. I waive my right to informed consent of treatment. This waiver applies only in the event that neither parent/guardian can be reached in the case of an emergency. *

I have read, understood & agree to the studio guidelines, regulations, terms & conditions and acknowledge all information to be true and correct. I hereby confirm that my child may enroll in dance classes at Barres and may take part in all activities. I will not hold the dance instructor or studio responsible for any injuries or loss, acquired during or in the classes, before or after class, art festivals, examinations or competitions. *

The teachers under the umbrella of Barres are of a standard which qualifies them to teach at the level of the class that you are participating in and, therefore, care is taken to provide you with quality and safe instruction. Nevertheless, accidents and unforeseen circumstances can occur. This agreement entails that you hereby give permission for yourself/your child to participate at Barres. I fully understand and accept that all activities shall be undertaken at mine/my child's own risk. I hereby indemnify and absolve Megg Geri and all staff against and from any claims whatsoever that may arise in connection with any accident, illness, damage or loss to the property of or death or injury to the person aforementioned. The owner and staff of the studio will nevertheless take all reasonable precautions for the safety and welfare of all students, to minimise any foreseeable risk of harm. As far as I know, he/she is physically capable of participating in physical activities and is in good health. Furthermore, I undertake to pay tuition fees by the due date and accept liability for payment of one term’s notice should my child terminate classes. This notice must be in writing. I acknowledge that there will be no credit or refund of fees should lessons be missed by my child. I have read and understood the studio guidelines, regulations, terms & conditions and acknowledge all information to be true and correct. *

Terms and Conditions: 1. A registration fee (new students) and a club fee (new and existing students) is payable at the start of the year. 2. Fees are compulsory for all classes, whether attended or not. 3. Class fees are on a per term basis (4 terms in the year). All fees are payable the first week of the term. A penalty of R40 will be charged on late payments, charged per week overdue. 4. Payments can be made via EFT, cash, or Snapscan. No refunds will be given. 5. Please make provision for additional fees for competitions, examinations, MMEL assessments, shows, private/extra classes, solo’s/dtq choreography, examination clothing and competition clothing. 6. Students who fail to attend any events (competitions, festivals, examinations, shows) at the allocated time will forfeit the fee paid. 7. All contemporary and acrobatics students (from school-going age) are encouraged to take part in two Acro/contemp classes per week. 8. All choreography remains the intellectual property of Barres (Megan Geri). 9. No choreography may be copied from any other studio, teacher, or academy. 10. No ideas from other dance academy, school, teacher, or student may be used in any of Barres choreographed pieces. 11. Tight fitting dance attire is compulsory for all dance classes as well as the correct footwear. 12. Examination clothing (studio wear) is compulsory for all examinations. 13. Studio clothing may only be bought through Barres or a recognised supplier. 14. The Studio t-shirt is compulsory for all festival and the show prize giving. 15. No lose jewellery is to be worn in dance classes as they cause distractions and can lead to injury. 16. Students are to be neatly groomed with hair fastened out of the face (with bobby pins). 17. All dancers must respect each other and obey the instructor’s instructions in the classroom. 18. The Studio has the right to arrange for another teacher, to give the lesson if the instructor is unavailable. 19. Students and their parents are liable for any damages caused to studio property. 20. Please leave all valuables at home. Barres is not responsible for lost or stolen property. 21. Lessons skipped must be paid for as class overheads are the same whether a student attends or not. 22. No pupil will be allowed to dance unless all fees have been paid, unless otherwise arranged. 23. Parents and siblings are only permitted during the last week of every term (watching week). 24. Rehearsals for the end of year concert, shows, competitions, eisteddfods, and examinations must be attended by pupils. 25. Regular attendance is important to the progress of all classes. 26. We reserve the right to cancel or postpone a class, if less than three students. 27. Class and rehearsal schedules may be subject to change. You will be notified as promptly as possible. 28. ONE TERM’S NOTICE must be given, including FULL PAYMENT, in the event that the student will no longer be continuing with classes. 29. Barres reserves the right to refuse service to anyone. 30. No extra classes will be given & no work will be revised for students who were absent for more than two classes. Dancers who miss more than 2 consecutive classes must arrange for a private lesson (at an extra fee) in order to catch up with the work they have missed. 31. Classes are not presented during school holidays or on public holidays. There may be extra classes, during public and school holidays as arranged beforehand by teacher. 32. It is understood and agreed that all students take classes at their own risk and no liability shall be undertaken by the proprietors for injury or loss of personal belongings. 33. In the event of a medical emergency, where a parent cannot be contacted, the guardian authorises Barres to seek medical advice for the child. 34. No responsibility will be taken for any students before or after their dance lessons. 35. All photographs or video's taken in class, competitions, festivals, or shows belong to the studio unless prior arrangements are made. 36. Barres reserves the right to use any photography or videos on the website and in marketing material. 37. No parents or students may take any photographs or video recordings of an classes, unless otherwise arranged. 38. The studio logo may not be used by anyone without permission. 39. I agree that my contact details may be used according to the POPIA Act and that I may be added to any WhatsApp groups associated with BARRES. *
